Patients are required to fill out paperwork with their first visit or if their existing information has changed.  This process can take 5-10 minutes.  As a convenience, you can print these pages off and fill them out before coming to the dental office. 

For a new patient, you will want to print out the forms 'Notice of Privacy Practices Acknowledgement', 'Welcome to our Practice' (Adult or Child), 'Authorization to Release Information', and the 'No Call No Show Policy'.

Simply bring them with you at your appointment.
